1. Send us an email with your full name and phone number.
2. An application will be mailed to you to complete.
3. You will need to provide us with 3 Letters of Recommendations.

4. Provide the Recommendation Form to your Recommender(s) to complete and to mail or fax back to us at: (541) 955-5233.
5. You will need to submit a non-refundable Application Fee of $75.
6. You will also need to submit a Commitment Holding Fee of $750 which will be refunded back to you after 6 months in the fellowship program. Should you decide not to attend the fellowship program AFTER you have been officially accepted into the fellowship program, you will forfeit this fee. If you have not been accepted into the fellowship program, you will receive the full refund within 10 business days.
7. We will then contact you to set up an interview.
8. A Fellowship Handbook will be issued to you at orientation day, or at the start of the fellowship program.
Applications will be reviewed by the Director/Coordinator of the fellowship program, as well as by the Administrators of Riverside Physical Therapy. The applications will be evaluated based on the following criteria:
